HOW TO APPLY FOR A GRANT

The process is simple.

  1. Click on the Guidelines and Instructions link below and make certain that you meet the qualifications sought by the Champaign County Memorial Foundation found on the page.
  2. Click on Grant Application link below and download and print out the application form. 
  3.  If instructed by a board member, click on the short form then download and print it out.
  4. Fill out the form, making certain that the signature of the board president/executive director and the treasurer have signed it.
  5. Mail the Application Form to the Champaign County Memorial Foundation at the address found at the end of the form. Mail to:
